Enclosed is a check or money order payable to "Connie Lenzen" in the amount of $ _______ as a retainer. (A retainer of 50% of the amount authorized is required. The final payment is due before the research report will be released.)

I ("Client") engage the genealogical services of Connie Lenzen, CG ("Researcher"), 10411 SW 41st Avenue, Portland, OR 97219, and I authorize her and her associates to conduct genealogical research on my behalf, for an amount not to exceed $_______ (time and expenses), as per my instructions.

I have provided to Connie Lenzen, CG, all pertinent information in my possession on the subject to be researched.

Connie Lenzen, CG, agrees to provide a detailed report covering all relevant sources checked during the research. Analysis of those sources, a list of suggested follow-up research, and a report of research time and fees will also be included.

The report will be sent to Client within a reasonable amount of time upon receipt of the retainer.

Client gives Researcher permission to submit such report and related files for renewal application purposes to the Board for Certification of Genealogists.

Client gives Researcher permission to use research findings in lectures, published materials.

The Researcher's work will be charged as follows: $_____ per hour plus out-of-pocket expenses including by not limited to photocopies, postage, repository entrance fees, database fees, and travel.

Research Services Include

• Review of information provided by you
• Formulation of the work plan
• Research and analysis
• Written report of findings, including negative findings
• Recommendations for future research
• Copies, transcriptions, or abstracts of documents, as appropriate

Retainer Fee

One-half of the negotiated research block; payable in advance. Other charges will be billed upon completion

Research time includes travel and telephone and email consultations with client.

Negative findings and unsuccessful searches are charged.

Reimbursable Expenses

Out-of-pocket costs including but not limited to postage, photocopies, parking, travel (if by car, at the standard IRS mileage rate), and repository entrance fees.
